MyChart Terms and Conditions

This web site ("Site") and related services are provided by OrthoVirginia to our patients subject to compliance with the terms and conditions set forth below. Please read the following information carefully. Your continued use of this Site will indicate your agreement to be bound by the terms and conditions set forth below. If you do not agree to be bound by these terms and conditions, please promptly exit this Site.

Your provider and OrthoVirginia are pleased to offer you MyChart. It is an online service that provides you with convenient access to portions of your medical record. It also offers a method of online communication with your physician and practice staff for such activities as prescription renewals, appointment scheduling and online messaging.

I. Privacy

Your privacy is of the utmost importance to us. OrthoVirginia will use your confidential medical information in order to provide you with health care services. OrthoVirginia will, at all times, maintain the privacy of your confidential medical information and will not disclose medical information about you unless you authorize the disclosure of your information or the disclosure of the information is permitted by law. Please review OrthoVirginia's Notice of Privacy Practices for a thorough description of how we gather, use and protect your confidential medical information. All messages sent and received within MyChart that contain medical information are subject to state and federal laws governing the security and confidentiality of medical records.

Certain highly sensitive information will not be released into your MyChart record.

II. MyChart Features

A. Electronic Messaging

MyChart should never be used for urgent matters. If you believe you are experiencing an urgent or life threatening health condition, please call your physician's office, go to an emergency room or dial 911.

B. Secure Communications

All communications between you and OrthoVirginia using MyChart occur over a secure connection. You are required to provide OrthoVirginia providers a valid e-mail address and it is your responsibility to keep this address updated if you change your e-mail account. You will receive internet e-mail messages notifying you that new information is available in your MyChart account. Although these e-mail messages will not contain any private medical information, the notification that new information is available by accessing MyChart may be information that you would not want others to know. Please take this into account when providing an e-mail address or using MyChart. All of the confidential medical information available to you in MyChart is protected and securely maintained by OrthoVirginia.

C. Activation Code

When you initially enroll in MyChart, you will be provided with an activation code. The activation code you receive will give you the opportunity to create a personal login name (called MyChart ID) and a password. Once this activation code has been used it will no longer be valid.

It is extremely important that you keep your login name and password completely confidential. Anyone with access to your login name and password will be able to access your confidential medical information. That person will also be able to read your messages and send new messages as if that person were you. It is your responsibility to prevent disclosure of your login and password, and to change your password if you feel that its security has been compromised. You may change your password at any time by logging into MyChart and choosing the "Change Password" option.

D. Features

OrthoVirginia's MyChart web portal provides the following services:

  • Ability to review, request or cancel your appointments
  • Ability to view selected portions of your medical record such as allergies, medications, histories, problem list and recent visit information
  • Ability to receive selected lab results
  • Ability to communicate with your health care provider via secure messaging regarding health concerns
  • Ability to request prescription refills
  • Ability to link to other sites that will provide you information

E. Minor Eligibility

An individual must be age 18 or older to request a personal account. Parents or legal guardians of minor children may request access to the minor child's account by following the terms outlined below.

Proxy Access

Individuals age 18 or older may request proxy access to another individual's MyChart account by completing the Proxy Access Form.

Once the form is completed by you and your designated proxy, please deliver in person, mail or fax to any OrthoVirginia office. Such access will only be granted to parties with parental rights or legal guardianship of the MyChart account holder or with the account holder's written authorization. A MyChart account will be activated for both the proxy and the account holder. If the proxy's legal relationship with the account holder changes, the proxy and/or the account holder must inform us immediately by calling the provider's office or sending written notice to the provider’s office.

III. Termination of Access

OrthoVirginia Providers may restrict or terminate access to MyChart at any time. The following is a list of reasons which may result in deactivation. The list is not inclusive and each circumstance will be evaluated individually.

  • No longer a patient of OrthoVirginia.
  • Automatic system deactivation will occur after a number of failed login attempts to access the secure portion of the MyChart system.
  • Inappropriate use of messaging or scheduling (ex: excessive, unnecessary, inappropriate content)
  • Abuse of proxy access
  • Pending legal issues with patient or parents/legal guardians
  • Inability to use features without constant assistance

IV. Confidentiality

Both federal and Virginia laws govern the confidentiality of patient health care records. We afford the same degree of confidentiality to medical information stored on MyChart as is given to medical information stored by OrthoVirginia in any other medium. We limit our employees' access and ability to enter or view information based upon their role in your care. Firewalls, passwords, encryption, and audit trails are further used to safeguard your information. We keep a record of the records released and note the time and date of access each time a patient accesses MyChart. We have taken steps to make all information received from our online visitors is as secure as possible against unauthorized access and use.

V. Security

The Site places considerable emphasis on the privacy of medical information and compliance with state and federal privacy laws. MyChart is configured to be secure from unauthorized access, but OrthoVirginia and the Site cannot be held responsible for:

  • Absolute security of all electronic communication transmissions between patient and OrthoVirginia
  • Unauthorized disclosure resulting from a user not logging out of an active session;
  • Unauthorized disclosure resulting from a lost or stolen user ID and password;
  • Unauthorized disclosure resulting from information printed by the user from MyChart;
  • Unauthorized disclosure resulting from personal computer settings or installed software products that may compromise information security.
  • Similar events outside the substantial control of OrthoVirginia or the Site.

VI. Links to Other Sites

OrthoVirginia may create links on the Site to other web sites that we think may be of interest to you or provide further information. OrthoVirginia makes no representations whatsoever about any other web site that you may access through this Site or any entity that may post its advertisements on this Site. When you access a non-OrthoVirginia site, it is independent from OrthoVirginia and OrthoVirginia has no control over the content on that web site. OrthoVirginia does not endorse or accept any responsibility for the content or use of any linked site. If you decide to access any third party web site linked to this Site, you do so entirely at your own risk.

OrthoVirginia has not reviewed the privacy policies of any web sites linked to or from the Site, and you should exercise care when visiting any other web site. We would like to hear from you if you find any web sites linked to or from the Site objectionable.

Please be aware that the MyChart functionality at OrthoVirginia, Bon Secours Health System, Carillion Health System, Inova Health System, the University of Virginia and other healthcare organizations is not integrated. Be sure you clearly maintain the difference between the user ID and passwords you may be using for other institutions providing you with their version of MyChart.
